HSC - 474 Medical Home Principles and Practice

This course introduces students to the concept of the medical home and other aspects of care coordination. Patients will investigate current medical home models throughout the country and the challenges faced by the changing healthcare landscape. Students will demonstrate an in-depth understanding of the various professionals working throughout the medical home, and how the model differs from previous attempts at coordinated care. Prerequisite: Departmental permission. Credit Hours: (4)
